The Digital Person main objective is to develop and validate a multidisciplinary AI-eCDSS for person-centred illness assessment and management in outpatient oncology care, as a means to improve satisfaction with telehealth. To achieve it, the specific objectives are established as follows: 1. To develop an integrated framework for illness assessment and management for patients undergoing outpatient adjuvant cancer treatment; 2. To harness AI-based trustworthy software to 1) predict wellbeing of people undergoing adjuvant cancer treatment in outpatient oncology, towards personalised illness assessment; and 2) integrate the predictive model with multidisciplinary guidelines for illness assessment and management. 3. To investigate the implementation of AI-enhanced telehealth within a person-centred clinical workflow in outpatient cancer settings. Assessment of Research Impact on Key Challenges The proposed research aims to contribute to a better life for the person undergoing outpatient adjuvant cancer treatment, during and after cancer treatment completion, by enhancing outpatient oncology care with a comprehensive and integrated assessment and management of patients' health and well-being. The project addresses a critical gap in traditional outpatient healthcare models, which may not fully capture the illness experience of the person beyond the measurable symptom. By incorporating predictive models of well-being that account for the unique and fluctuating needs of cancer patients, the project enables the implementation of more proactive and personalised care strategies. Furthermore, the research innovates by incorporating an AI-eCDSS that account for the patient’s subjective experience in outpatient oncology care, offering tailored, evidence-based recommendations to improve clinical decision accuracy. Additionally, the project explores the integration of AI-enhanced telehealth services according to person-centred care principles, this ensures continuous and accessible support for patients, regardless of their geographic location, further democratising quality care for all. Innovation and Ambition Beyond Current Paradigms The proposal's objectives ambitiously extend beyond current practices. It introduces an integrated illness assessment and management framework, shifting from traditional single-symptom or disease-oriented focus to a comprehensive, multidisciplinary approach, advancing personalised cancer care significantly. Introducing a predictive wellbeing model for outpatient oncology marks a ground-breaking advancement, diving into underexplored areas of patient care. Unlike traditional models mainly focused on adverse events, this approach prioritises patient perceived needs and potential challenges, addressing a significant gap in oncology care. The development of a person-centred AI-eCDSS is a pioneering step, blending AI with clinical and PCC principles. Its integration into PCC workflows will transform illness assessment and management in telehealth, fostering narrative sharing, collaborative goal-setting, and shared decision-making. This method aims to balance the focus on both the illness experience and measurable symptoms, substantially improving telehealth-mediated PCC. Exploring the integration of AI-enhanced telehealth into person-centred workflows offers a new perspective on incorporating advanced technology into care, signifying a substantial interdisciplinary development.
